Non-Profit Application Fee — for a trainer that is employed by a non-profit organization. A nonprofit organization is an entity identified by the IRS as tax exempt under section 501(c)(3)

For Profit Application Fee — for a trainer that is employed by a for-profit organization.

Unique nine-digit number assigned by the Internal Revenue Service (IRS). This is only required if the trainer works for a non-profit organization.
